Camp Forms

Click on the appropriate form to be taken to it. Some forms are in PDF format. You must have the free Adobe Reader software installed on your computer to view and print.

• Online Registration Forms:
  • Scout Registration/Permission Forms — Remember, no printed forms will be handed out this year!
  • Adult Registration Form — For adults attending camp without sons (or found out after registering their son that they could sneak away from their boss for a couple days).
• All medical forms are due Tuesday, July 2!! Parts A and B of the Annual Health and Medical Record form are required with parent signature for youth participants. All participants, regardless of age must have an annual physical from a medical professional for events exceeding 72 consecutive hours. You must have a doctor fill out part C. As far as I know, vaccination records, prescription medicine lists, etc. may be stapled to the form.
