Summer Camp Information

Camp Forms

• Camp Program Registration Form — This form is available only on this website. See that page for more information and instructions.
• Pig Roast Registration Form — for non-campers. If family members will be attending please download, fill out, and return with appropriate fees. All week campers have the fee built into their registration fee. Due Tuesday May 29.
• Troop 53 T-shirt & Hoodie order form — order yours by May 15!
• All medical forms are due Tuesday, May 29. Parts A and C of the Annual Health and Medical Record form are required with parent signature for youth participants. All participants, regardless of age must now have an annual physical from a medical professional for events exceeding 72 consecutive hours. You must have a doctor fill out part B. As far as I know, vaccination records, prescription medicine lists, etc. may be stapled to the form.
